Return from leave of Absence/Readmissions


Undergraduate F-1 students who took a leave of absence for two quarters or more must apply for readmissions to UCLA through the Registrar’s Office and be readmitted to UCLA in order for an I-20 to be issued from DCISS. Students who have been academically dismissed from UCLA must also apply for readmissions. Please see UCLA Registrar’s Readmission procedures. After you have been readmitted to UCLA, please follow the steps outlines below to obtain an I-20 from DCISS.

Return from Leave of Absence

F-1 students who took a personal leave of absence for one quarter or an extended period of time and had their SEVIS record terminated by UCLA, must request a new I-20 from DCISS to re-enter and resume their studies at UCLA. Please follow the steps below to obtain a new I-20:

Step 1: Obtain a FedEx Account Number

DCISS will ship your new I-20 overseas via FedEx. You will need to obtain a FedEx Account Number for DCISS to process your shipment. Obtain a FedEx Account Number in one of the following ways:

Via Online: www.fedex.com

Via Phone: 1-800-Go-Fedex (1-800-463-3339)

Via In-Person: Go to your local FedEx Center

Step 2: Complete DCISS I-20 Request form and Obtain Supporting Documents

Complete top portion of I-20 request form. Obtain proof of financial support (bank statements, etc.) and a copy of your passport photopage.

Step 3: Submit I-20 Request to DCISS

Submit your I-20 Request form, FedEx Account Number, and supporting documents listed in Step 2 to DCISS in one of two ways:

Via Mail: ATTN: F-1 Unit, 106 Bradley Intl Hall, 417 Charles Young Dr. West, Los Angeles, CA, 90095.

Via E-Mail: Send your I-20 Request form and scanned copies of supporting documents via E-Mail to our SEVIS Coordinator: Sam Nahidi

Your new I-20 will be issued 5 business days after the date your request was accepted. Our office will use your FedEx Account Number and ship your I-20 to you overseas to the shipping address you have indicated. Fastest estimated shipping time is 3 business days.

Step 4: Pay SEVIS Fee and Apply for an F-1 Visa from a U.S. Embassy/Consulate (if applicable)

Pay the mandatory SEVIS fee. Please print out and retain a copy of your SEVIS fee payment receipt. If you have been outside the U.S. for 5 months or more then you will need to apply for a new F-1 Visa. Please schedule an appointment for a visa interview at a U.S. Embassy/Consulate in your home country.

Step 5: Re-Entry to the U.S.

You may enter the U.S. with your new UCLA I-20 and F-1 visa up to (but no earlier) than 30 days before the start date indicated on your new UCLA I-20.
